What do I need to do to become a new patient?
- Request an appointment for an initial consultation with Lyn by emailing email@example.com or calling 617-566-1170. In order to set up an appointment, a credit card (Mastercard or Visa) will be needed to hold your appointment. If you would like to use this card the pay for your appointment, kindly bring it in with you. If you would like to use another form of payment (cash, check or alternate credit card) on the day of your appointment, please bring it in with you. Your credit card will be charged the full fee for the appointment if you miss, change or cancel without a minimum of 24 hours notice.
- Please be sure to print out all new patient forms and bring the completed forms with you to your appointment. It is not necessary to email them to the office beforehand, as Lyn will review them with you in person. If you cannot print them at home, please arrive 15 minutes early so you can fill them out before your appointment so as not to use valuable treatment time.
- Be sure to review all office policies, especially the late arrival and late cancellation policies.
- Bring a notebook or ipad so you can take notes during your session with Lyn.
